About the Role:
In this role, you will provide care to Cityblock members as part of our innovative model. You will be working on the Mobile Integrated Care team which operates 24/7 virtual urgent care and can deploy Community Care Clinicians (typically paramedic, EMT, RN, or MA trained) to members homes in response to urgent member clinical and behavioral needs.
Together with our team of virtual emergency medicine PAs/NPs, you will develop treatment plans and address acute needs via phone or video, and will be able to deploy our Community Care Clinician ED at Home field team to implement assessment and treatment at the member’s home. You will be responsible for collaborating with and overseeing advanced practice providers (NPs/PAs) during virtual visits, working closely with the Community Care Clinicians to plan next steps, as well as closing the loop with the member’s primary care team.
Requirements for the Role:
- You are an MD or DO Board Certified in Family Medicine, Internal Medicine +/- Pediatrics, or Emergency Medicine with an unrestricted license to practice
- You possess a valid State Controlled Substance license and DEA license
- You have a minimum of 3 years work experience in high volume emergency department
- You have a license to practice in one or more (preferably 2+) of the following states (MA, DC, NC, NY, CT, OH) and are willing to obtain licenses in all other existing and future Cityblock markets; Cityblock will help in this ongoing process
- You are passionate about the growing role innovative technology can play in improving access to care and care delivery
- You are an independent self-starter, a leader, and a strategic thinker who is excited about the big picture of whole health, and the ongoing evaluation and iteration of our care model
- You have experience working within an interdisciplinary team
- Work full-time with flexibility for 12 hour shifts, including some weekend day and holiday day call coverage
