About the role

The Supervisor of the Population Health Team is a leadership position that oversees the daily duties and responsibilities of the Population Health teams.  The position oversees quality metrics and coordinates programs to support organizational goals. The role also oversees caregivers on the team providing daily support for issues and concerns and in designing specific staff workflows.

Required qualifications:

  • Bachelor's Degree and Preferred field: Healthcare related degree. Or equivalent educ/experience
  • 2 years of experience in management/supervisory position.
  • 2 years of experience utilizing disease registries or similar databases.
  • 2 years of experience with electronic medical records and/or computerized billing systems.
  • 2 years of experience in utilizing databases and spreadsheets.

Preferred qualifications:

  • Master's Degree in Healthcare Administration of Healthcare related degree    Or equivalent educ/experience
  • 2 years of experience of direct care in primary care.
  • 2 years of EPIC, Care Manager, software experience.

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
