What You'll Do:
- Implement and facilitate person-centered resident engagement programs under the direction of the Well-Being Coordinator, tailored to individual needs, abilities, interests, and routines.
- Observe, assess, document, and contribute to resident care and service plans using EMR systems, supporting holistic well-being through a person-directed approach.
- Support resident participation in group and one-on-one engagement opportunities by fostering creativity, inclusion, adaptability, and a safe, dementia-friendly environment.
- Collaborate with interdisciplinary team members, including Nursing, to ensure continuity of care, resident safety, and promotion of resident rights.
- Utilize best-practice research, community resources, and engagement technology platforms while following established engagement and training calendars.
- Demonstrate professionalism, compassion, and flexibility; participate in required training, maintain certifications, assist with team onboarding, and perform related duties as assigned.
What You'll Need:
- High School Diploma or GED (required)
- Must be 18 years of age or older (required)
- Minimum of 1 year of experience in a Senior Living, Dementia Care, or Rehabilitation setting (required)
- Minimum of 1 year of experience working directly with individuals living with dementia (required)
- Current and active CNA or related healthcare certification/licensure in the State of Illinois (preferred)
- Computer literacy, including experience with EMR or similar documentation systems (required)
