Shift/schedule: Part Time (24 hrs/wk)
GENERAL SUMMARY
Works under the supervision of the Manager of Social Services. The SW Student Discharge Planner's primary job function is to provide support to patients and their families, with a focus on discharge planning and making appropriate referrals based on the patient’s age and needs. Performs other duties as assigned.
QUALIFICATIONS
Education:
• High School Diploma or successful completion of an equivalent High School Exam Required
• Bachelor's Degree required
• Enrolled in Master's Degree Program, or completed Master's Degree in Social Work required.
Licensure:
• BLS within 90 days of hire required

JOB SPECIFIC DUTIES AND PERFORMANCE EXPECTATIONS

1. Arranges continuing care post discharge including coordination of services with community resources.
2. Completes assessments and follow-ups for patients/families.
3. Serves as a liaison between patients, physicians, organizations, and outside agencies.
4. Responds to high-risk screenings and physicians' consults according to criteria.
5. Maintains accurate record keeping in patient's chart and departmental statistics.
6. Participates in team meetings, non-departmental meetings and on-going hospital committees.
7. Participates in department Quality Improvement programs.
8. Maintains skills through continuing education and current knowledge of community resources.
9. Arranges discharge planning needs by meeting and speaking with patients and families in addition to collaborating with providers, nursing, therapy and other skilled disciplines.
10. Serves as a liaison between patients, families, physicians and advanced care practitioners, organizations, and outside agencies.
