POSITION/BUSINESS TITLE: Family Support for Student Success Specialist

REPORTS TO: Program Supervisor 

CCSI SERVICE LINE: Program Management 

Full-Time- non-exempt- 40 hrs

LOCATION: Syracuse, NY

ABOUT THE POSITION: Join the Family Support for Student Success Program and work with local schools and the Onondaga County Department of Children and Family Services to ensure child safety and boost student success. 

Requirements

  

SPECIFIC RESPONSIBILITIES INCLUDE:

  • Bridge the gap between home and school by working with families, teachers, support teams, other service agencies, and administrators.
  • Support families to overcome challenges affecting their child's academic performance.
  • Ensure the safety and welfare of children.
  • Collaborate with school staff and  community providers to remove barriers to student success.

  

Home Visits and Family Support:

  • Conduct bi-weekly home visits for assigned families.
  • Coordinate support and services for up to twelve families with intensive needs.
  • Provide goal-driven service planning, case management, and linkages to reduce barriers.
  • Assess safety and risk for all children in the caseload at every contact.
  • Facilitate Service Plan and Review meetings as scheduled.
  • Coordinate transportation for service planning as needed.
  • Manage urgent client situations with flexibility.


School Collaboration:

  • Serve as a resource and team member within the School Intervention Team.
  • Collaborate with school teams on assessments for assigned students/families.
  • Bridge the gap between home and school by working with families, teachers, support teams, other service agencies, and administrators.
  • Facilitate interventions and problem-solving with students, families, school personnel, and community partners.


Professional Conduct and Compliance:

  • Provide strength-based and culturally competent services.
  • Maintain professional relationships with school staff, service providers, and community organizations.
  • Adhere to Agency policies and procedures.
  • Complete the NYS Child Welfare Foundations program within the first six months of employment.
  • Complete all NYS OCFS mandated assessments, progress notes, and documentation.
  • Report to the assigned school three days a week and to Rock West/remote location two days a week as designated by the Program Supervisor.
  • Perform other assigned duties and tasks.
  • Other related job duties as assigned.

EDUCATION AND EXPERIENCE: 

  • Bachelor’s degree or equivalency.
  • Ability to satisfactorily complete required background screenings: criminal, DMV, and NY State Central Registry. (see CCSI's equivalency chart)
