Job duties:

• Triage phone calls and triage patient care along with field staff.
• Communicate with field staff, physicians and patients while also performing and completing workflow in our EMR, HomeCare HomeBase.
• Intake and referrals for St Joseph Hospice.

**HomeCare HomeBase “back office” experience required**

Qualifications:

• Minimum of 3 years as a Registered Nurse (RN) required, 1 of these 3 years must consist of full-time experience in direct patient care in a hospice setting
• Current compact nursing license and CPR certification required
• Knowledge of EMR (Homecare Homebase) is required
• Compassionate RN with great time management skills
• Telephone triage call center experience a plus
