White Oak of Burlington is a beautiful 160-bed long term and rehabilitation facility located at 323 Baldwin Rd. Burlington, NC 27217

We are currently seeking a full-time Social Services Director to join our amazing team.

Essential Functions:

Maintains policies and procedures to assure that the medically related emotional and social needs of the residents are met/maintained on an individual basis. Interviews residents and families to obtain history relevant to medical problems and planning. Helps resident and family to adjust to admission into the facility environment. Acts as liaison between medical and nursing staff, residents, relatives and appropriate outside agencies. Assists in coordination of discharge planning for residents. Maintains Social Work Progress Notes by recording all significant contracts made in the course of performing the service.

Education:

Bachelor’s Degree in Social Work, Psychology, Sociology or related field is required.
