Summary:

The provision of these job responsibilities are mandated by the Centers for Medicare & Medicaid (CMS) for Medicare-approved transplant centers. As a member of the transplant team, the social worker must possess specialized clinical knowledge and advanced clinical skills to help in the areas of assessment, treatment of medical, patient/ caregiver navigation, mental health and behavioral disorders, and discharge planning.

Responsibilities:

Provides crisis intervention for patients and family members.Interviews and assesses patients and/or family, caregivers, and/or legal representatives. Performs comprehensive psychosocial assessments for pre- post and living donors to determine transplant candidacy and continue to work with patient and family through all phases of the transplant process during inpatient and outpatient and end of life. Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.May supervise and/or train new staff, students, and interns.Attend multidisciplinary rounds as mandated by CMS for all Medicare-approved transplant centers and identifies appropriate resources for financial, insurance, transportation, housing, healthcare, mental health, spiritual services, and education about diagnosis and transplantation.Assist patient in the comprehension of complex instructions, resolve conflicts, or facilities conflict resolution, have strong organization/ priority setting skills, must be able to learn and apply large amount of technical and procedural information, must be able to multi-task, and must be able to communicate effectively, verbally, and in writing.Design and facilitate support groups and other programs.Develop behavioral agreements with patient and/or families that addresses adherence and commitment to treatment plan. Assist with patient fund-raisingConsults and collaborates with multidisciplinary transplant providers as mandated by CMS for all Medicare-approved transplant centers to promote, monitor, and evaluate patient's ability to understand and adhere to treatment plan and lifestyle changes. Make appropriate referrals to community agencies, such as DCF, APS, SSA, CMS, disease specific resources as deem necessary.Participation in transplant selection committee meetings

Additional Job Information:

Complexity of Work: Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful and fast pace environment and take appropriate action. Required Work Experience: At least 2 years post MSW experience in related field required.Previous experience working in hospital setting.
