Under the supervision of the Director of Home Health, position is responsible for day to day management of Home Health Care clinical services, including supervision of clinical staff, patient and personnel assignments, coordinating patient care, assuring that patient needs are continually assessed, and assuring the development, implementation, and updates of the individualized plan of care. Maintains 24 hour accountability for clinical services of Home Health. Participates in the development of nursing and organizational strategic plan. Systematically enhances the quality and effectiveness of nursing and therapy practice, and the delivery of patient and family centered care. Promotes a caring, healing environment for patients, families and staff. Provides clinical suport to staff. Supervises staff in their delivery of care and evaluation of results. Adheres to ethical principles of practice as defined by the Nurse Practice Act and the ANA Code of Ethics for Nurses. Addresses breaches in clinical or ethical standards. Responsible for provisions of services consistent with the mission and goals of the system, meeting or exceeding quality outcomes, regulatory and financial goals.
Education –Bachelor's degree in clinical field required. Master's degree preferred.
Experience – Minimum of five years experience in a home health or health care setting is required. Thorough knowledge of home health regulations policies and procedures is preferred.
Licensure/Certification – Current license to practice as RN, PT, OT, SLP, Audiologist, SW, or Physician in the State of Maryland required. Professional certification in Nursing Leadership or area of clinical specialty preferred. Current BLS in compliance with policy required. For use of Personal Vehicles: Access to a vehicle and current driver’s license from any state is required. Acknowledgement with signature to “Attestation A” of Meritus Vehicle Management Use and Control policy, which includes guidelines for personal insurance coverage requirements.
Knowledge/Skills/Abilities – Ability to make public presentations to groups, teach one-on-one to staff with varying learning styles, advanced computer skills including use of Microsoft Office, and the ability to learn new software programs are required. Ability to apply the standards of professional clinical leadership with compassion and empathy. Must possess high level of organizational and communication skills.
