Key Responsibilities:
✅ Serve as the primary point of contact for referral sources (e.g., acute care hospitals, physicians, case managers) regarding patient admissions to the rehab facility.
✅ Evaluate potential patients for medical rehabilitation suitability by reviewing clinical data and collaborating with the rehab clinical team.
✅ Educate patients, families, and healthcare providers about the benefits and services offered by the rehab facility.
Minimum Requirements:
✔️ Bachelor’s Degree in Nursing, Healthcare Administration, Social Work, or a related field.
✔️Two years of experience in a clinical, case management, or healthcare marketing role.
✔️ Possess clinical license in one of the following: RN, PT, OT, SLP, or LCSW
✔️ Certified in BCLS (Basic Cardiac Life Support)
✔️ In-depth understanding of medical rehabilitation processes and patient needs
Preferred Qualifications:
✔️ Prior experience as a liaison or admissions/discharge coordinator in an inpatient rehab facility (IRF) or skilled nursing facility (SNF).
