World Relief seeks a part-time Health Navigator to support the Health Services team coordinating medical appointments and expanding mental health referrals for refugee clients, including administering mental health screeners to identify needs. This position will also assist eligible refugee clients with accessing disability benefits.

Role & Responsibilities

• Provide health-related case management to a small caseload of refugee clients
• Administer mental health screeners to refugee clients to identify needs and determine appropriate referrals
• Assist Health Services Manager in establishing partnerships with local mental health providers
• Schedule and coordinate details for mental health appointments for refugee clients
• Assist eligible clients in navigating the process for accessing disability benefits and coordinating any necessary follow-up
• Assist Health Services team in coordinating other medical appointments, as needed
• Provide health education to clients on a variety of topics related to navigating health systems, nutrition, mental health access, etc.
• Assist in the development and implementation of a transition process for clients to begin assuming responsibility for the scheduling and coordination of their own appointments
• Communicate regularly with Health Services team and caseworkers in the Initial Resettlement and Extended Services departments on mutual clients
• Conduct regular home visits with clients, requiring consistent travel throughout the Fox Valley area with clients living in communities from Appleton to Oshkosh
• Transport clients to medical appointments, as needed
• Maintain confidential documentation, case notes, and client data utilizing grant program databases

Knowledge, Skills, & Abilities

• Excellent communication skills, both written and oral
• Experience working cross-culturally and have some working knowledge of refugee health-related topics
• Experience navigating complex healthcare systems and advocating on behalf of vulnerable populations needing access to healthcare, preferred
• Fluent in Arabic, Dari, Dinka, Kinyarwanda, Spanish, Swahili, or Ukrainian, preferred

• Ability to pass a motor vehicle record check and maintain a clean driving record
