- Collaborate with medical and mental health teams to deliver safe, effective patient care.
- Interpret and communicate complex psychosocial needs; model caring and professional practices.
- Assess psychosocial risk factors through evaluation of prior functioning, support systems, and reactions to illness.
- Counsel patients and families regarding emotional, social, and financial impacts of illness or disability.
- Access and mobilize community and family resources to meet patient needs.
- Conduct mental health assessments upon physician referral and provide therapeutic interventions.
- Facilitate complex discharge planning, including guardianship, foster care, adoption, and mental health placements.
- Serve as the primary point person for cases involving child abuse/neglect, elderly abuse, domestic violence, and institutional abuse.
- Liaise with protective services, law enforcement, schools, probation, and other agencies.
- Utilize crisis de-escalation, interviewing, and intervention techniques to support high-risk and dangerous situations.
- Lead clinical interventions during emergencies to ensure patient safety and stability.
- Education: Master’s Degree in Social Work required.
- Licensure: Permanent LSW licensure required; LCSW licensure preferred.
- Certifications: De-escalation certification required within 6 months of hire.
- Experience: 3-5 years preferred in mental health, child and family services, adult protective services, Veteran’s Administration, or substance abuse.
- Additional Skills & Knowledge:
- Knowledge of disease impact, human behavior, and crisis intervention techniques.
- Understanding of legal mandates related to protective services.
- Principles of growth and development across the lifespan.
- Strong crisis de-escalation and intervention skills.
- Ability to develop and maintain strong relationships with community agencies, law enforcement, and healthcare teams.
- Proficiency in Microsoft Excel and Word.
