About the Department
 

The University of Chicago has developed a new internal medicine practice for patients at high risk of hospitalization which is staffed by Comprehensive Care Physicians (CCPs) who care for these patients in both the inpatient and outpatient setting. Each of about 6 CCPs has a panel of 200-250 patients each. These patients have a high degree of acuity and require specialized care from a multidisciplinary team. The team includes nurses, a nurse practitioner, social workers, program coordinators, and a community health worker, who meet daily to provide the best possible care for their patients.

Job Summary
 

The CCP Program Nurse will assist physicians in the CCP clinic in patient care activities. They will be responsible for managing clinic flow, including obtaining vitals, rooming patients, and appropriately allocating exam rooms to physicians. They will facilitate care coordination between multiple internal and external resources and also assist in assuring seamless transitions of care between inpatient and outpatient settings. They will be responsible for answering patient phone calls and assisting with medication refill requests and DME (durable medical equipment), and home health service referral requests. They will also manage incoming and outgoing faxes and inbasket messages, and stocking patient education materials. CCP nurses need to possess a strong customer service orientation and a commitment to excellence and be highly self-motivated and team oriented while working in a fast-paced, multi-tasking environment with seriously ill and complex patients. They must be able to collaborate with all members of the clinic staff to ensure that patients are treated in a safe, comfortable, and caring environment.

Responsibilities

• Organize daily clinic work flows for physician and other providers to ensure optimal patient flows.

• Manage clinic flow, including obtaining vitals and rooming patients.

• Answers patient phone calls and triages these as appropriate.

• Review clinic schedule daily to proactively identify patient needs and ensure that necessary resources are available at the time of the visit (patient education, social work assistance, procedures).

• Using the CCP dashboard, tracks patients appointments and provides personal reminder calls to high risk patients.

• Coordinates care between inpatient case managers, home health agencies, CCP clinicians, and pharmacies as necessary.

• Provides health related education to patients and families.

• Has a solid understanding of human behavior and performance; individual differences in ability, personality, and interests; learning and motivation; psychological research methods; and the assessment and treatment of behavioral and affective disorders.

• Has a solid understanding of the information and techniques needed to diagnose and treat human injuries, diseases, and deformities. This includes symptoms, treatment alternatives, drug properties and interactions, and preventive health-care measures.

• Performs other related work as needed.

Minimum Qualifications
 

Education:

Minimum requirements include a college or university degree in related field.
