Job Summary
Responsibilities
Organize daily clinic work flows for physician and other providers to ensure optimal patient flows.
Manage clinic flow, including obtaining vitals and rooming patients.
Answers patient phone calls and triages these as appropriate.
Review clinic schedule daily to proactively identify patient needs and ensure that necessary resources are available at the time of the visit (patient education, social work assistance, procedures).
Using the CCP dashboard, tracks patients appointments and provides personal reminder calls to high risk patients.
Coordinates care between inpatient case managers, home health agencies, CCP clinicians, and pharmacies as necessary.
Provides health related education to patients and families.
Has a solid understanding of human behavior and performance; individual differences in ability, personality, and interests; learning and motivation; psychological research methods; and the assessment and treatment of behavioral and affective disorders.
Has a solid understanding of the information and techniques needed to diagnose and treat human injuries, diseases, and deformities. This includes symptoms, treatment alternatives, drug properties and interactions, and preventive health-care measures.
Performs other related work as needed.

Internal medicine ambulatory experience.
