Job Summary
The Phase I Cardiac Education Nurse will be responsible for the planning, oversight, implementation, and follow up of the elements contributing to the patient education process. The educator will address all aspects of patient wellness continuum including prevention and lifestyle management. The educator will also address applicable disease processes including medication management, diet, activity, stress management
Essential Functions

- Demonstrates the following for all patients:
- Understanding of anatomy, physiology, and pathophysiology
- Understanding of diagnostic testing, pharmacological management
- Knowledge of medical, surgical, and interventional treatments for Cardiovascular and Pulmonary disease states
- Identifies educational needs of patients, families, and significant others
- Identifies and refers patients for outpatient follow up/ classes as appropriate
- Provides appropriate educational materials including booklets, handouts, and online resources
- Understanding of the implications of exercise for individuals with Cardiovascular and Pulmonary conditions
- Assesses patients for the ability to maintain compliance for all aspects of treatment.
- Ability to assess, evaluate, and document the response of patients, families and significant others to education provided..
- Formulates an education plan for each patient utilizing principles for adult learning
- Evaluates data provided by invasive and noninvasive testing/procedures to include in the patient education plan
- Participation in interdisciplinary assessment and planning
- Assesses patient status and risk for readmission
- Participation/Program Development
- Participates in quarterly staff meetings with Director and Medical Director
- Participates in goal development and tracking for the department and program
- Participates in educational offerings for the community
- Participates in educational offerings for the staff and support departments
- Participates in the development and implementation of policies, procedures, and protocols
- Participates with onboarding of new staff as applicable to the service
- Serves as a mentor and resource as appropriate
- Contributes to Quality improvement efforts within the department
Job Qualifications
Education
- Bachelor of Science in Nursing required
Experience
- Minimum of two years education/teaching experience preferred
- Critical Care experience preferred
Licensure
- Current CA RN licensure
- BLS – Basic Life Support from the American Heart Association.
- ACLS - Advanced Cardiac Life Support Certification from the American Heart Association preferred
Knowledge, Skills, and Abilities
- Demonstrate current knowledge of program development, and adult learning theory
- Strong communication skills with the ability to communicate across all levels of the Hospital
- Strong presentation and public speaking skills
- Autonomous – ability to be effective with minimal supervision
- Strong organization skills with the ability to prioritize workload.
- Must possess effective interpersonal and leadership skills
- Advanced Computer experience preferred
- Ability to work various shifts as assigned
- Ability to practice CICARE
