Comforting Families.</strong></p> <p> </p> <p><strong>Schedule:</strong></p> <p><strong>Friday 12:00 p.m. - 8:00 p.m.</strong></p> <p><strong>Saturday 10:00 a.m. - 6:00 p.m. </strong></p> <p><strong>Sunday 10:00 a.m. - 6:00 p.m.</strong></p> <p> </p> <p>We are looking for a compassionate Hospice Licensed Practical Nurse (LPN) to join our team. Reporting directly to the Administrator or Executive Director, you will play a vital role in delivering hands-on hospice care to patients and families in their homes, assisted living, or facility settings. Your dedication will help provide comfort and support during the most critical moments of life.</p> <p> </p> <p><strong>As a Hospice LPN, You Will:</strong></p> <ul> <li><p>Provide direct nursing care services to hospice patients, supporting both patients and their families.</p></li> <li><p>Assist the interdisciplinary team in assessing patient/family needs and contribute to the execution of individualized care plans.</p></li> <li><p>Report any changes in patient conditions promptly to the Registered Nurse and care team.</p></li> <li><p>Deliver empathetic care aligned with each patient’s unique plan of care.</p></li> <li><p>Serve as a trusted resource to families, offering support during difficult times.</p></li> </ul> <p><strong>Responsibilities:</strong></p> <ul> <li><p>Provide skilled bedside nursing care under the supervision of a Registered Nurse, following established nursing protocols and the Nurse Practice Act.</p></li> <li><p>Monitor patients actively and document any changes in condition, reporting as necessary.</p></li> <li><p>Administer medications and treatments according to the plan of care.</p></li> <li><p>Participate in infection control practices and standard precautions.</p></li> <li><p>Support bereavement activities and closure contacts with families as assigned.</p></li> <li><p>Participate in on-call duties and respond to patient needs during scheduled visits or emergencies.</p></li> <li><p>May assist with death pronouncements per state law and under RN supervision.</p></li> <li><p>Collaborate closely with hospice aides and other team members to ensure comprehensive care delivery.</p></li> </ul> <h2>About You</h2> <p><strong>Qualifications:</strong></p> <ul> <li><p>Graduate of an accredited Practical Nursing program.</p></li> <li><p>Current LPN/LVN license in the state of employment.</p></li> <li><p>Minimum 2 years of LPN experience in hospice, oncology, long-term care, or home health; 3-5 years preferred.</p></li> <li><p>CPR certification required.</p></li> <li><p>Valid driver’s license and current automobile insurance.</p></li> </ul> <p><strong>Skills and Knowledge:</strong></p> <ul> <li><p>Understanding of hospice care principles and the unique needs of terminally ill patients and their families.</p></li> <li><p>Excellent patient assessment and clinical skills.</p></li> <li><p>Strong communication and documentation abilities.</p></li> <li><p>Familiarity with regulatory guidelines including Medicare, Medicaid, CMS, and JCAHO standards.</p></li> <li><p>Ability to work independently and as part of an interdisciplinary team.</p></li> </ul> <p>Compensation may vary within the salary range provided based on several factors, including but not limited to a candidate's geographic location, experience, education, skills, licensure, certifications, internal equity, and department equity.
