About the role

Description

The Oversight and Attending Physician, Long Term Care provides clinical oversight and direct physician care for residents of assigned skilled nursing and long-term care facilities. This role works collaboratively with advanced practice providers, facility leadership, nursing staff, and the interdisciplinary care team to support high-quality, timely, and compliant resident care.

Responsibilities:

  • Serve as the attending physician for assigned residents and facilities.
  • Provide clinical collaboration or supervision for assigned advanced practice providers in accordance with applicable state requirements, written agreements, and organizational policies.
  • Complete required physician services, including admission evaluations, history and physical examinations, regulatory visits, acute visits, and other medically necessary encounters.
  • Round on-site at assigned facilities at least every other week, with weekly rounding preferred.
  • Remain available by phone or video for consultation regarding changes in condition, medication management, resident safety concerns, goals of care, and escalation or transfer decisions.
  • Review and sign orders, documentation, care plans, and other clinical records as required.
  • Complete periodic case or chart reviews and provide clinical feedback as required by law, payer standards, collaborative agreements, or organizational policy.
  • Collaborate with facility leadership and the interdisciplinary care team to reduce avoidable hospitalizations and support appropriate care within the facility.
  • Complete documentation accurately and within organizational expectations.
  • Other duties as assigned.

Typical Time Commitment:

  • On-site rounding at least every other week for acute and routine long-term care needs, with approximately two hours anticipated per visit.
  • Availability by phone or video for clinical consultation between on-site visits.
  • Additional availability for required facility or clinical meetings, as appropriate.

Work Location: Assigned long-term care facilities, with remote availability for consultation

Requirements

Required Qualifications:

  • MD or DO degree with an active, unrestricted physician license in the applicable state.
  • Current board certification in geriatrics, family medicine, or internal medicine.
  • Current DEA registration and applicable state-controlled-substance authority.
  • Knowledge of CMS requirements and long-term care regulations.
  • Ability to meet applicable collaborative or supervisory requirements.

Preferred Qualifications:

  • Experience in geriatrics, post-acute care, skilled nursing, or long-term care.
  • Experience supporting nurse practitioners or physician assistants.
  • Familiarity with medically complex residents and transitions of care.
  • Previous experience with electronic medical records and remote clinical consultation.

Physical Requirements:

  • Ability to travel to and work within assigned long-term care facilities.
  • Ability to remain standing, walking, or seated for periods necessary to complete resident rounds and documentation.
  • Ability to perform repetitive keyboard and computer tasks.
