Location: Walnut Creek, CA (On-site)
Schedule: Monday–Friday, Day Shift
Job Type: Full-Time
Pay: $40–$50/hour
Experience Level: 1+ year preferred

This vital, on-site role helps ensure seamless coordination of patient care, supports field clinicians, and strengthens communication between patients, families, and physicians.

 Key Responsibilities

• Coordinate and support care for home health patients
• Track, clarify, and input provider orders into EMR
• Communicate with clinicians, patients, families, and providers
• Organize and monitor medical supplies & maintain OSHA standards
• Assist in new staff orientation
• Perform audits and general administrative tasks

How you'll be successful:

• California LVN License (Required)
• 1+ year of Nursing Experience (Home Health preferred)
• Strong communication skills—verbal and written
• Comfort with high-volume phone calls
• Excellent organizational and multitasking skills
• Proficiency with Apple devices and EMR/scheduling software
• Working knowledge of medical terminology
• Customer service mindset focused on patient care
• BLS Certification (Preferred)
