Southeast Primary Care Partners is seeking a highly motivated Director of Population Health Strategy to join our leadership team. This role is instrumental in driving the adoption of our innovative care model while shaping program growth and strategy. The Director will work closely with leaders across Clinical/Physician Services, Population Health, and Clinic Operations to oversee the successful execution of initiatives relating to clinical practices, technology enablement, value-based care programs, and market expansion. As a leader, you will initiate high-priority projects, foster collaboration with stakeholders, and drive sustainable performance improvements to elevate care delivery across the organization. This position calls for strategic vision, operational effectiveness, deep knowledge of value-based care, healthcare technology, and regulatory standards, including compliance with OSHA requirements. The Director will also implement and oversee workplace health and safety standards, ensuring physical and mental health needs for teams are met.

Key Responsibilities
Clinical Program Strategy & Implementation
- Develop scalable playbooks for launching, implementing, and optimizing clinical programs across the organization.
- Design and deliver targeted educational initiatives for physicians, clinical staff, and administrative teams, focusing on implementing clinical programs to improve outcomes.
- Guide strategic conversations with physicians to set pathways based on performance metrics and provide ongoing feedback to ensure sustainable success.
- Collaborate with clinical operations to grow clinical programs, emphasizing patient engagement and improved clinical outcomes.
Population Health Program Strategy
- Develop strategies for proactive patient panel management, including outreach, risk stratification, and tailored interventions for high-risk patients.
- Track and analyze performance measures tied to value-based contracts, offering insights that inform decision-making and drive measurable improvements.
- Engineer patient engagement strategies that address unique regional or community needs, ensuring alignment with market nuances.
Technology Enablement
- Identify, assess, and implement cutting-edge technology opportunities to support the organization’s population health vision and foundations.
- Tailor innovative technology solutions to enhance value-based care models, risk management, and operational efficiencies.
- Oversee the rollout and adoption of key technology platforms, preparing teams with strategies to maximize their impact at both patient and organizational levels.
New Market Growth
- Support the development and execution of growth strategies for emerging markets, aligning initiatives with organizational objectives and market trends.
- Partner across teams to identify target populations and regional opportunities to develop sustainable operations and patient-focused solutions.
Physical Requirements
- Ability to stand, sit, or walk for extended periods while performing job duties at clinics or corporate office environments.
- Perform tasks requiring repetitive motion (e.g., typing, reviewing data, attending meetings).
- Lift and carry up to 25 pounds occasionally, as needed.
- Navigate clinical and office settings safely in compliance with OSHA standards.
