We are currently seeking a case manager, you will join our Aging & People with Disabilities (APD) team in our office in Hillsboro, Oregon.

The services provided help people access the supports they need to live their best lives — in the communities they choose and at every stage of life.

Summary of Duties
As a Case Manager, you will:
Conduct interviews with participant(s) and collateral contacts to determine initial and ongoing eligibility for long-term care, healthcare, social services, and protective services.
Utilize state systems, assessment tools, and available resources to document, review, and update case information and long-term care services.
Assess individual needs, safety, and risk to develop detailed, person-centered service plans and determine appropriate services, referrals, and resources.
Process applications, inquiries, and eligibility information within established policy timeframes, ensuring accurate documentation, forms, and communication of eligibility decisions.
Contact participants or collateral contacts monthly to monitor service plan and safety needs.
