Registered Nurse – RN- Health Coach Consultant (Hybrid)
Location: Winston-Salem, NC (OR within a 30-mile radius)
Schedule: Full-Time (40 hours/week) | Remote/Hybrid
We are seeking an experienced Registered Nurse (RN) with a strong background in Medical-Surgical nursing to provide Chronic Condition Education. This role combines remote work with occasional onsite responsibilities for a dedicated client in Winston-Salem, NC or within a 30-mile radius.
Key Highlights:
- Hybrid Role: Primarily remote with potential onsite work up to five days per week as needed.
- Coaching Focus: Individual telephonic coaching (1:1) and/or group coaching sessions.
Language Skills:
- Spanish-speaking RN preferred
Work Schedule:
- Onsite: Monday–Friday, 8:00 AM–4:30 PM EST (flexibility may be required).
- Remote: Standard business hours supporting 8:00 AM–4:30 PM EST, plus:
- One evening shift per week (12:30 PM–9:00 PM EST).
- One late Friday rotation approximately every 12 weeks.
- No weekends required
Role Overview:
The Health Coach Consultant uses a collaborative process of assessment, planning, implementation, and evaluation to engage and educate members, promoting informed decisions that support optimal health outcomes.
Responsibilities:
- Member Assessment & Planning
- Assess members using clinical tools and data review; conduct comprehensive evaluations of needs and benefit plan eligibility for integrated programs/services.
- Utilize assessment techniques to determine health literacy, technology capabilities, and readiness to change.
- Develop and monitor individualized care plans in collaboration with members and providers to meet health goals.
- Enhancing Medical Appropriateness & Quality of Care
- Apply and interpret clinical guidelines, health/wellness management plans, policies, and regulatory standards to ensure appropriate utilization of services.
- Use a holistic approach to consult with supervisors, Medical Directors, and others to overcome barriers to care.
- Identify and escalate quality-of-care issues through established channels.
- Member Engagement & Coaching
- Utilize motivational interviewing and influencing skills to maximize member engagement and promote lifestyle/behavior changes.
- Provide coaching, education, and support to empower members to make informed health decisions and adopt healthy behaviors.
- Deliver up-to-date healthcare information to facilitate understanding of health status and treatment options.
- Encourage active participation in healthcare decision-making with providers.
- Monitoring, Evaluation & Documentation
- Track progress toward goals and adjust care plans as needed.
- Document all interactions and care plans in compliance with internal policies, regulatory, and accreditation guidelines.
Required Qualifications
- Active, unrestricted North Carolina compact RN license with ability to obtain additional state licenses as needed.
- Minimum 3 years of clinical RN experience in adult medical-surgical nursing.
- High-speed internet access with hardwired connection.
- Proficiency in Microsoft Office (Word, Outlook, Excel, Teams, PowerPoint).
- Experience with electronic medical records (EMR) documentation.
- Strong communication skills, including comfort with public speaking.
- Ability to work independently and collaboratively.
- Willingness to travel onsite to Winston-Salem as needed.
Preferred Qualifications
- Telephonic chronic condition coaching experience.
- Fluency in Spanish.
- Virtual group coaching experience via webcam.
- Managed care experience.
- Familiarity with Epic EMR functionality.
Education
- Associate Degree in Nursing (ADN) required.
- Bachelor of Science in Nursing (BSN) preferred.
