The VNA Health Group is seeking a Per Diem Physical Therapist to join our team in Monmouth County. In this role, you will deliver in-home physical therapy services, providing personalized, compassionate care that helps patients recover and improve their mobility in the comfort of their own homes.

Responsibilities

What You Will Do:

    • Perform in-home physical therapy evaluations and treatments tailored to each patient.
    • Develop and implement individualized care plans in collaboration with physicians and care teams.
    • Document patient progress and adjust care plans to meet evolving needs.
    • Educate patients and their families to support continued progress and improved outcomes.
    • Maintain high standards of care and ensure adherence to all safety and clinical protocols.

Qualifications

What You Will Need:

  • License: Valid New Jersey Physical Therapy License.
  • Experience: 2–3 years of clinical experience required; community-based or homecare experience preferred.
  • Skills: Strong communication, assessment, and computer documentation skills.
  • Other: Valid NJ driver’s license and reliable transportation.

Working Conditions/Physical Demand:This is a field-based role requiring travel throughout Monmouth County to patient homes. Includes use of clinical tools, technology, and regular interaction with patients, families, and interdisciplinary teams.
