UC Health is hiring for Pharmacy Resident for UC Medical Center

Under the supervision of the Director of Pharmacy, and according to standard procedure the Pharmacy Resident (Pharmacy Practice or Specialty) is a pharmacist in training who provides comprehensive pharmaceutical care through integrated drug distribution, clinical services and teaching.  The resident provides services consistent with other services provided by pharmacists at UC Health. The resident training program is jointly supported by the corporate department of UC Health Pharmacy Services.

Patient Population:

• Engages in population appropriate communication.

• Involves family/guardian in the assessment, initial treatment, and continuing care of the patient.

Direct Patient Care:

• Provides Pharmaceutical care by assisting physicians or providing direct patient care by:
  • Reviewing patient medication history
  • Educating patients on drug therapy
  • Establishing desired outcomes
  • Assessing patient demographics
  • Developing pharmacotherapeutic plans
  • Determining alternatives for achieving outcomes
  • Developing drug monitoring plans
  • Implementing new drug regimens
  • Monitoring patient outcomes in concert with medication use
  • Documenting medication related issues in patient medical record
  • Interpreting laboratory data in context with medication use
  • Reviewing patient medication profiles and making recommendations for drug therapy management
  • Monitoring pharmacokinetics of drug therapy
  • Monitoring and preventing potential adverse, drug-drug, and drug-food interactions
  • Preventing and monitoring for medication errors
• Completing patient specific medication consults
• Participates in patient care rounds focusing on drug therapy issues.
• Monitors drug therapy to evaluate appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication, and drug interactions.
• Provides pharmacokinetic consultations as required, specifically addressing drugs with narrow therapeutic indices.
• Detects, monitors, documents, and reports adverse drug reactions (ADRs) and medication errors (MEs).
• Promotes the use of the formulary by converting non-formulary orders to formulary, when possible, but coordinates procurement of non-formulary drugs when necessary.
• Counsels patients on admission or discharge medications as necessary.
• Participates in the drug therapy management of medical emergencies, responds to Code Blue and Rapid Responses, and prepares medications as needed.
• Facilitate discharge planning, including engaging Medication Access Services if needed, to ensure a smooth transition to alternate site of care or home.
