County Managers are responsible for overseeing the day-to-day operations of the Community Living Support Programs and other assigned programs for Premier Community Supports in the assigned County. This job onboards and manages Individuals receiving Community Living Supports and the Direct Support Professionals who provide care, while maintaining a favorable reputation and working relationship with County Case Managers and other interested community partners.

Essential Duties:

1. Relationship Management: Learn and manage complex relationships with Individuals-Served/families, interested community members, PCS staff, and government agencies.

2. Individuals-Served Onboarding: Review Individuals-Served referrals from case managers. This entails filling out paperwork, requesting any documents, and making an appointment.

3. Person-Centered care for Individuals-Served: Identify supports needed and assign DSPs, maintain consistent contact, ensure documentation is current such as Medication Administration Records, Individual Support Plans, Service Agreements, Safety Plans and Protocols, etc.

4. Individuals-Served/DSP: Match the needs of the Individuals-Served/families while selecting DSPs with matching skills.

5. County Financial Status: Increases Premier’s market share of assigned county and increases profitability via onboarding new Individual’s- Served/families and DSPs, maximizing DSP hours worked and other means.

6. DSP Staffing: Identify, assist, and manage DSPs with tasks, communication with Individuals-Served/families, scheduling, training on approach, create a work environment conducive to Premier staff retention, arrange for and provide backup, maintaining consistent contact, educating on policies, etc.

7. Compliance: Manage and resolve any issues by reviewing Premier policies and Government Regulations with Individuals-Served/families, case management, and co-workers/supervisors.

8. Auditing Records: Ensures that all DSP and Individuals-Served/families’ information, Service Agreements, Protocols, Safety information, documentation, training, work availability is accurate and up to date.

9. Office Management: Manage phone calls, walk-ins and meetings, emails, texts from all parties, attend assigned meetings, and efficient time management.

10. Incident Reports: To ensure that incidents are reported in a timely manner; to clarify and accurately respond to incidents. To cooperate or assist with all investigations; internal or external.

11. Other Duties: Ability to perform and complete any additional tasks/projects as assigned.

4. Handles crisis situations effectively and ethically according to Premier

Policies and Government Mandates.

5. Writes clearly, timely and accurately, including report writing, record keeping, and other documentation.

6. Contributes to building a positive and productive relationship with work team and Individual’s-Served/families.

7. Ability to pass a drug test upon request.

8. Ability to learn and implement CPR, First Aid practices, and understanding ISPs.

9. Effectively manages time by prioritizing, planning, scheduling work activities, ability to deal with frequent change, delays, or unexpected events.

10. Anticipates future changes and creates plans to best fit the situation.

Requirements

Education and Experience:

1. Bachelor’s Degree or equivalent Preferred

2. 4 years in Human Serv
