Description

Hospice of the Piedmont is currently seeking a full-time Social Worker to join our caring and compassionate team to provide direct casework and counseling to patients and their families during end-of-life care. Requirements

  • Provides psychosocial support and social work services to hospice patients and families in the inpatient unit.
  • Completes psychosocial assessments, casework, and counseling.
  • Develops care and discharge plans for inpatient facility patients.
  • Collaborates with the interdisciplinary team and participates in meetings.
  • Prepares accurate documentation per Hospice standards.
  • Adheres to NASW Code of Ethics and confidentiality policies.
  • Updates knowledge of community resources and facilitates referrals.
  • Participates in evening/weekend coverage.
  • Maintains quality and productivity standards.
  • Applies knowledge of emotional, social, and spiritual needs related to death and grief.
  • Facilitates bereavement support groups.
  • Supports staff and volunteers in coping with job-related stress.
  • Engages in agency and community programs for learning and performance enhancement.
  • Communicates with outside agencies.
  • Manages placements, discharges, and level of care changes.
  • Takes responsibility for advance directives, bereavement referrals, APS referrals, funeral planning, volunteer referrals, and accurate documentation in the EMR.


Qualifications

  • MSW degree from an accredited school or Bachelor's in a related field with MSW supervision.
  • Preferred: Current state licensure (LCSW).
  • Two years of social work experience, with one year in healthcare.
  • Knowledge of death, dying, bereavement, and psychosocial factors in terminally ill patients.
  • Understanding of social work principles, counseling techniques, hospice philosophy, and group dynamics.
  • Skills in counseling, interviewing, and communication.
  • Knowledge of resource/support agencies and accessing services
  • Must be able to work in home environments that have difficult working conditions including, but not limited to cramped space, pets, dirt, allergens, smoke, no chairs 
  • Able to drive short/long distances
  • Driver’s license, personal automobile, and current automobile liability insurance
