Job description:

Assist patients with prescribed occupational/physical therapies. In collaboration with others, carry out an individualized, comprehensive, and patient centered Plan of Care in accordance with current applicable Federal, State and local standards, guidelines and regulations as may be directed by the Physical or Occupational Therapist, Rehabilitative Services Manager, and/or Director of Patient Care Services.

Required Qualifications:

• Within 30 days of hire: National Provider BLS - American Heart Association 
• 3 months Long-term care or related geriatric and/or rehabilitative experience.

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
