Job Summary:This position performs care management and coordination to service providers, community partners, caregivers and guardians of children and youth in out of home care statewide throughout their involvement of DCS, including Medicaid and non-Medicaid eligible children and supports the delivery of integrated (physical health and behavioral health) health care management and resolution of concerns pertaining to eligibility, enrollment, access to health care services and navigation within the Department of Child Safety Comprehensive Health Plan (DCS CHP) or alternate Arizona Health Care Cost Containment System (AHCCCS) health plan in pursuit of the Arizona Twelve Principles for children's system of care.
This position may offer the ability to work remotely, within Arizona, based upon the department's business needs and continual meeting of expected performance measures.

Job Duties:- Responds to community requests for support or care management for child-specific or system related needs to promote timely and effective operation of DCS CHP and Mercy Care DCS CHP within standards of care. Develops professional partnerships with internal and external colleagues.
- Provides technical assistance and support to CHP team regarding coordination of care and information systems. Provides technical assistance, support and advocacy for delivery of physical and behavioral health care delivery amidst DCS processes (i.e. Courts, Caregivers) for children involved with DCS. Documents efforts.
- Manages System of Care projects coordinating action plans amongst DCS CHP team members.
- Maintains up-to-date knowledge of AHCCCS, DCS and DCS CHP policies and procedures and participates in data-driven performance management to advise quality and network activities.
- Participates in activities beyond regular work hours on a rotational basis; handle incoming telephonic inquiries from members, providers, caregivers and DCS staff during off-hours.
- Other duties as assigned as related to the position.
Knowledge of:
- Applicable laws and regulations
- Government (Medicare, Medicaid, Supplemental Security Income (SSI), and Social Security Disability Insurance (SSDI) and insurance benefits including AHCCCS Medical Policy and Contractors Operations Manuals
- New regulations on federal, state and local levels as well as practice guidelines and standards of practice
Skills in:
