Schedule: Monday - Thursday
- Monday - Wednesday: 9:00 a.m. - 5:30 p.m.
- Thursday: 9:00 a.m. - 2:00 p.m.

Job Summary: The Housing Solutions Case Manager supports Mercy House clients in securing and maintaining permanent housing using the Housing First Model. Under the direction of the Housing Solutions Manager, this role provides case management and housing advocacy to participants in Rapid Re-housing and Homeless Prevention Programs. Responsibilities include assessing clients, creating and monitoring housing plans, and tracking progress toward housing goals.
Essential Duties and Responsibilities:
Program Oversight
- Maintain assigned caseload of approximately 17-25 clients
- Attend Family Support Team (FST) and/or multidisciplinary team (MDT) meetings
- Provide comprehensive case management, including intake, planning, goal setting, referrals, and monitoring
- Use harm reduction strategies and trauma-informed care
- Practice Motivational Interviewing
- Maintain GIRP-format case notes
- Educate tenants, advocate, and refer clients to services
- Develop and monitor individualized case plans and financial/housing goals
- Conduct regular client meetings and home visits
- Ensure timely and accurate HMIS data entry and maintain up-to-date case files
- Track client progress in master logs
- Perform habitability inspections and recertifications
- Help clients address housing/employment barriers and connect to community resources
- Assist in housing searches and foster relationships with landlords
- Mediate landlord/tenant issues as needed
- Participate in Coordinated Entry System meetings and submit required updates
Team Tasks
- Collaborate with staff, interns, and volunteers to fulfill the agency mission
- Attend meetings, supervision, and trainings
- Participate in monthly Housing Solutions team meetings
- Coordinate with leasing agents and other departments
- Foster positive team relationships
- Adapt to a fast-paced environment and shifting workloads
Administration
- Maintain accurate client documentation (emergency plans, case notes, call logs, etc.)
- Upload all necessary backup documentation
- Support program activities and data collection
- Build partnerships with external agencies
- Ensure confidentiality and compliance with HIPAA
- Use computers and software efficiently
- Respond promptly to calls and emails
- Prioritize tasks and deadlines
Qualifications
Requirements:
Education and Experience
- Bachelor’s in social work or related field, or a minimum of four years of experience in social or human services required
- Experience with homeless populations strongly preferred
General
- Valid CA driver’s license, insurance, and reliable vehicle are required
- Willingness to travel for client visits in various settings
- Strong organizational, communication, and interpersonal skills
- Ability to manage crisis situations and maintain confidentiality
- Proficiency in Microsoft Office and internet tools
- Multilingual skills (Arabic, Farsi, Spanish, Vietnamese) highly valued
- Able to work evenings/weekends as needed
