SUMMARY OF POSITION:

Pay Rate: $31.00/hour

Schedule: Per Diem (Picks up shifts as needed)

Location: Greenfield, MA

This LPN position will be held in both a Residential home setting as well as a Day Program setting. The LPN would be providing care to a small group of individuals, and would have the same individuals on each shift. The Per Diem Licensed Practical Nurse (LPN) reports to the Nurse Manager for all nursing related duties and the Residential House Manager (RHM) / Day Program Manager for all programmatic duties.

The LPN will attend person supported medical appointments whenever possible, provide nursing care during on-duty hours, establish plans of care for each person in the residence and facilitate health education and training of the direct care professionals in order to ensure the health and safety of the people served. This position will be compensated on an hourly basis.

Requirements

• Current and valid registration as a professional nurse under the Massachusetts Board of Registration in Nursing.
• At least one year of full time working experience as an LPN.
• Must be 21+ years old, have a valid driver’s license, daily use of a car on site and be available by telephone for daily contact.
