Warde Senior Living in Windham, NH is looking for an experienced full-time Director of Social Services/Admissions to join our leadership team. 

Warde a 39-bed Assisted Living community and a 32-bed skilled nursing facility offering short and long-term care in a warm, supportive atmosphere where residents are cared for according to their individual needs. Job Responsibilities;

· Schedule and conduct tours for prospective families and applicants 

· Interview residents/families to obtain social history.

· Complete admission paperwork with the resident and/or appropriate family member

· Participate in discharge planning, development and implementation of social care plans and resident assessments

· Involve the resident/family in planning objectives, approaches, and goals for the resident and invite resident/family members to social service programs and Care Plan meetings when available and appropriate

· Participate in survey (inspections) made by authorized government agencies; and review and develop a Plan of Correction for Social Service Department deficiencies noted during survey and provide a written copy of such to the Administrator

Requirements

• Bachelor’s degree in Social Work, or similar field required.
• Minimum of three years of supervised social work experience in a healthcare setting working directly with individuals
• Supervisory experience preferred
• Long term care and assisted living experience preferred
