- Provides effective screenings and crisis intervention to at-risk individuals and families engaging in homeless services in the community.
- Provides assessment of the financial and insurance needs, completes the SSI/SSDI, Outreach, Access and Recovery (SOAR) screening tool for adults and children.
- Assists individuals in completion of social security benefit applications using on-line application process.
- Develops medical summary reports.
- Becomes the individual’s representative using the SSA 1696.
- Tracks SOAR outcome data using approved SOAR Online Application Tracking (OAT) forms and updates database.
- Provides support and encouragement to the individual throughout the application process.
- Serves as the single point of accountability for ensuring continuity of care related to SOAR activities.
- Provide transportation, when necessary and appropriate.
- Provide appropriate referral and linkage to community resources; to include employment, insurance, and other subsidies, as eligible.
- Will be working in the community and meeting individuals in their living environment.
Qualifications
- Bachelor’s degree from an accredited university or college in human services.
- One (1) year experience in human service field preferred.
- Ability to pass a level II Background clearance and drug test.
- Valid FL drivers’ license, insurance, and safe driving record.
- CBHC New Hire Orientation (NHO) completed within first two (2) months of employment.
- Must maintain high standards of ethical and professional conduct, while adhering to agency policies and procedures.
- Ability to use a computer.
- Attention to detail.
- Ability to work independently and as part of a team, in collaboration with other community partners.
- Ability to manage stressful situations and display appropriate work demeanor and boundaries.
