About the role

Summary: 

Baystate Health is seeking motivated pharmacists who want to grow with us in Ambulatory Care and Retail Settings.  We believe that pharmacists can have a major impact on the care of our patients.  We currently have an opening for a Full Time Flex Pharmacist who will have the opportunity to serve in multiple roles in the organization, with a focus on providing Ambulatory Care Services (Coumadin) as well as traditional retail pharmacy.  With the rapid expansion of Ambulatory Services anticipated, the candidate should have an expressed interest in providing in-office or telephonic managed patient care.  Ambulatory Services will be provided via Collaborative Drug Therapy Management (CDTM) Protocols as well as policies and procedures.

Baystate Health currently has 3 Retail Pharmacy locations and is supporting Ambulatory Pharmacy Services at additional sites across Western Mass.  In this role, time will be flexed based on business needs between providing ambulatory services and retail services.  The successful candidate will have the opportunity to have robust patient interactions and work to solve complex problems as part of a team with the patient and providers in the Health System.  Key responsibilities include:

  • Provide direct patient care in a variety of settings (office, telephonic, retail)
  • Provide customized pharmaceutical care plans for patients
  • Prescribe/recommend appropriate drug therapy and monitoring plans, in collaboration with the patients’ providers in the ambulatory setting
  • Accurately dispense medications in the retail pharmacy setting and counsel patients

As a Clinical Pharmacist II (Flex), you have a critical role at the forefront of delivering genuine, authentic care for our patients. In addition to supporting the Pharmacy Manager in leading and directing your Pharmacy Technician Support Staff, you are accountable for supporting the management, oversight and operation of all aspects within your pharmacy.

Required Qualifications

  • Active Pharmacy License in Massachusetts
  • Not on the DEA Excluded Parties List
  • Immunization Certification through an accredited organization (e.g., APhA)
  • No pending felony charges or convictions for criminal offenses involving controlled substances

Preferred Qualifications

  • PGY1 Pharmacy Residency Training (Either Acute Care or Community)
  • PGY2 Ambulatory Residency Training
  • Board Certification (BCACP preferred)
  • CACP credential
