Job Description
Provides health service coordination for assigned patients and families.Experience
- 5 years of clinical experience
Preferred Qualifications
- Experience working with infants, toddlers, and young children in community settings
- Experience in care coordination and case management
- Bachelor of Science in Nursing
- Graduate degree, e.g., Master of Science in Nursing or Master of Public Health
Education
- Graduation from an accredited school of nursing
Certification Summary
- Licensure as a Registered Nurse in the single State of Georgia or Multi-State through the Enhanced Nurse Licensure Compact

- Ability to build relationships with community residents, community agency staff, pediatricians, family physicians, and other healthcare providers

- Excellent oral and written communication skills and a high degree of professionalism

- Flexibility to work occasional weeknight or weekend hours
Job Responsibilities
- Works to ensure that assigned children from birth to age 18 have access to and participate in a medical home.
- Provides support to parents of newborns as needed and assists with continuation of care within assigned facility.
- Acts as primary point of contact between children and families at assigned facility, primary care physician, and/or other health providers as needed.
- Ensures children are receiving immunizations based on national and state schedules, developmental and health screenings, and referrals for additional services as needed.
- Coordinates cares for chronically ill children.
- Coordinates joint meetings between family, primary care physician, facility staff, and other providers as necessary.
- Serves as link between primary care physician, specialty providers, and other ancillary providers to ensure service delivery for assigned children.
- Provides health promotion activities and education for parents at assigned facility, including workshops and trainings.
- Supports children and families at assigned facility and acts as advocate on their behalf.
- Maintains databases/logs as appropriate and provides reports on a routine basis.
- Works with primary care and specialty case staff to ensure appropriate, timely, and effective follow-up coordination is provided and documented in patient's medical record.
