Location: Frenchtown, NJ
Work Schedule: Full-time: (Saturday-Wednesday; 3:00pm-11:00pm)

What You’ll Do:
• Supports consumers in achieving personal care goals and addressing ongoing challenges.
• Transport consumers using company van as needed.
• Adheres to the Adult Mental Health Rehabilitation Services standards (AMHRS).
• Respond promptly to consumer concerns, ensuring high satisfaction.
• Educate consumers on medications, track logs, and conduct monthly audits.
• Coordinate services based on individual needs.
• Deliver high-quality care in line with Individual Rehabilitation Plans (IRP).
• Accurately document services within required timeframes using electronic health records.


Job Requirements:

• Bachelor’s degree in mental health, psychology, sociology, social work or a related field from an accredited college or university OR
• A combination of one (1) or more years of college, plus related life or work experience equal to four (4) years OR
• High School or GED plus four (4) years of related work or life experience.
• Intermediate computer skills with the ability to complete documentation in an Electronic Health Record (EHR) system.
• Valid driver’s license and an acceptable driving record.
• Strong, effective verbal and written communication skills.
• A compassionate, flexible, and client-focused approach to care.
