We are excited to offer the role of Virtual Physician Collaborator for CenterWell Medical Group, a new virtual care delivery group within CenterWell Senior Primary Care, Humana’s Primary Care Organization.



This group will provide virtual wraparound services to support patients when they cannot get in with their own PCP – closing gaps in care, ensuring timeless access to important medications and solving urgent clinical needs. This Virtual Physician Collaborator shares in our vision to innovate on primary care for the next generation of seniors and is eager to support excellent care each and every day! We will be serving patients in Texas and we are looking for an MD/DO who is licensed in this state. This role is paid hourly and will focus on the collaborative relationship through the support of physician assistants and advanced practice registered nurses for approximately 5 hours/month.

Responsibilities:

· Act as a collaborating physician for nurse practitioners or physician assistants, including completing regular chart review and coordinating monthly meetings.

· Be available to act as a resource for the advanced practice provider to ensure exceptional patient care.

· Follow policy and protocol defined by Clinical Leadership.

· Meet with clinic leaders about quality of care, review of outcome data, policy, procedure, and records issues.

· Assure personal compliance with licensing, certification, and accrediting bodies.

Required Qualifications:

· Current and unrestricted medical license in the state of Texas.

· Graduate of accredited MD or DO program of accredited university

· Board certification or eligible to become certified (ABMS or AOA) in Family Medicine, Internal Medicine, or Geriatric Medicine

· Active and unrestricted DEA license

· Medicare Provider Number

· Medicaid Provider Number

· Excellent verbal and written communication skills

· Superior patient/customer service skills

· Demonstrate a high level of skill with interpersonal relations
