A Lead Navigator contributes to AHF by  providing coordinated, client-centered support that ensures individuals are successfully connected to critical health and social services. This includes maintaining and monitoring referral processes for PrEP, HIV care, STI treatment, housing, and insurance navigation to ensure accuracy, consistency, and successful outcomes. The role also provides guidance to Community Health Workers, resolves referral-related issues, and supports data reporting efforts to strengthen the quality and impact of AHF’s prevention and care services.

 

A Lead Navigator enhances the reputation of AHF by  ensuring that every client referred to care experiences a smooth, respectful, and effective transition into services. By maintaining high standards of communication, follow-up, and documentation across all referral types, they demonstrate AHF’s commitment to quality, accountability, and client-centered care. Their responsiveness, professionalism, and collaboration with both internal staff and external partners reinforce AHF’s reputation as a trusted provider that prioritizes the needs of the community and delivers seamless access to life-saving services.

AHF has a collaborative organizational structure where staff are accountable to multiple leaders. 

 

The Lead Navigator will work as a part of a close knit team to ensure clients receive seamless, coordinated care across all stages of the referral process. They will collaborate with Community Health Workers, outreach staff, and leadership to troubleshoot referral challenges, improve workflows, and strengthen connections to services like PrEP, HIV care, STI treatment, housing, and insurance support. By contributing their expertise and maintaining open lines of communication, they help foster a supportive team environment that prioritizes accountability, shared learning, and high-quality service delivery across the Prevention Department.
