The Role:

Work as an effective member of the RHH Social Work Women’s and Children’s and broader multidisciplinary teams in the planning, implementation and evaluation of Social Work services for patients and their families by:

  • Assessment of complex psychosocial needs of the patients and their families.
  • Specialist interventions for patients and families in matters relating to psychosocial, health and mental health needs.  including adjustment to illness, rehabilitation and case coordination of Child Protection and Family Violence matters.
  • Information, advocacy and referral for patients consistent with their needs.
  • Active leadership in case conferences, family meetings and case management as required.
  • Effective working relationships with community-based service providers.

You will contribute to the assessment of complex psychosocial needs and provide specialist interventions, advocacy and coordinate referrals for TPRS clients.  Your duties may include assisting families with adjustment to illness and case coordination of complex needs, demonstrating leadership and building effective working relationships with stakeholders.

This is a fantastic opportunity for a social worker with extensive clinical experience ideally in a women’s and children’s environment and/or rehabilitation, who would like to be part of an exciting new service.

You will need:

  • Extensive experience as a social worker, including clinical competence in acute settings and in the continuum of Women’s and Children Service areas.
  • Experience in the assessment, intervention and management of patients and their families with complex psychosocial, health and mental health needs, including issues relating to complex and high-risk pregnancy, chronic and life-threatening diagnosis, rehabilitation, child protection and family violence.
  • Demonstrated leadership in complex decision making, including use of relevant legislation to support patient safety and wellbeing.

Details of appointment:

Permanent part time, shift worker position, working 38 hours per fortnight, commencing as soon as possible.
