About the role

Description

Our community outreach specialist will support our initiatives by developing a more extensive awareness and support services to educate and engage residents/individuals on health care services offered at UCHC. This role will assist with continuity of care and provide essential resources for retention and growth.


Responsibilities:

  • Assist with outreach, scheduling appointment-related information and work directly with patients through inbound/outbound telephone calls. 
  • Convene and facilitate regular check-ins with the Quality Director to evaluate outreach progress and outcomes.
  • Address patient questions related to the Remote Patient Monitoring (RPM) Program via telephone to improve patient engagement
  • Ensure timely of completion of RPM referrals by scheduling enrollment appointments with the RPM Care Coordinator
  • Maintain accurate documentation of RPM patient activity and follow-up status in Google Sheets and eClinicalWorks
  • Organize and facilitate activities to educate and collect input from community members and key stakeholders.
  • Represent UCHC in Marketing and promote all services that are provided by the health center, including mobile clinic outreach
  • Develop and distribute outreach materials and brochures
  • Collaborate with enrollment coordinators to facilitate health program enrollments/patient portal.
  • Generate data reports at regular intervals throughout the year to check for accurate and complete data collection. Send messenger messages to patients and the UCHC community. 
  • Understand program goals, monitor progress toward meeting these goals, and communicate this progress to program leadership on a regular basis
  • Other duties as assigned by the Quality Director

Requirements

  • High School diploma or GED is a must
  • Enthusiasm for helping others and making an impact on the community we live in by solving problems and providing creative alternatives
  • Passion for people and confidence speaking in front of small or large groups
  • Preferred experience in health education, outreach, and HEDIS measures. 
  • Experience conducting outreach in culturally and economically diverse communities
  • Experience using a variety of strategies to collect community
  • Bilingual REQUIRED: English/Spanish 
  • Flexible work schedule; required to work both in clinic setting and field-based
  • Computer proficient with Google G-Suite
  • Current valid California Driver’s License and Insurance Certificate and regular access to a reliable vehicle
  • Commitment to good attendance and punctuality

  • Experience working with not-for-profit agency/organization
