About the role

The Team

Royal Hobart Hospital (RHH) is Tasmania’s largest hospital and major referral centre. As the major clinical teaching and research centre, RHH collaborates with the University of Tasmania and other institutions. All our services work closely together to focus on delivering excellence in healthcare to achieve improved outcomes for our consumers, our workforce, and the Tasmanian community.

The Hospital@home program sits within the Department of Sub-acute, Aged & Community Services and provides care to a range of medical, surgical and sub-specialty patients who require medical management that can be delivered in the community environment.

The Hospital@home programme was started in April 2020 and currently has 30 beds, 18 acute & 12 subacute Geriatric Evaluation and Management (GEM) beds. We also have a Rapid Response service that assesses community patients to prevent hospital admission. The service runs 7-days per week.  At present we assess around 220 patients a month. Our average length of stay of 5-6 days for acute patients and 13 for subacute.

Our case mix and the age of our patients is broad. Greater than 60% of our patients are admitted directly from the community avoiding the hospital entirely. The rest are admitting from the Royal Hobart Hospital, both inpatient wards and the emergency department. These patients are then cared for in community health centres, their homes, or residential aged care facilities.

The interdisciplinary service is comprised of Medical staff, Nurse Practitioners, Nurses, Pharmacists, and Allied Health Professionals, who all work together to deliver effective clinical care.

  • Actively pursue improved outcomes for patients by participating in relevant teaching and research.
