This is highly independent and responsible Advanced Practice Registered Nurse position with the Family Planning/STD Clinic. This position is part of a multidisciplinary team that ensure proper care and treatment to clients who are seen for the following services: Family Planning, STD, HIV, PrEP, Refugee Health, School Health, Community Outreach, TB, and Immunizations.   

 

Family Planning: Provide  contraceptives, male and female services, perform contraceptive procedures, client education and counseling and documentation of services in the HMS Electronic Health Record. Also ensures review of laboratory results by physicians followed by timely case management, follow-up treatment and referral as indicated for the client.

Performs STD exams, evaluations and management as deemed appropriate. Performs Family planning exams-including history and physical, complete assessment and plan. Provide variety of birth control options to those who medically qualify.

Performs school physicals for all ages needing school entry. These are simple routine physicals. Those needing specialized care will be referred out after consultation with MD. Participates in the orientation and staff development programs for personnel assisting in patient care identifying improvements and adjustments needed to ensure program effectiveness and efficiency.  Provides technical assistance and guidance to the program nursing staff through comprehensive review of charts, records, referrals, etc. with a primary focus on patient outcomes and quality care.  Identify alterations needed, implement corrective actions as required.

Refugee Health: Conducts health history, and initial health assessments for Refugee Health clients to determine services indicated for eligible clients within 90 days of arrival to the U.S. as outlined by the Office of Refugee Resettlement. The assessment includes, but is not limited to vital signs, head-to-toe assessment, postural assessment, vision and hearing screenings, mini mental health screenings. Will obtain and maintain nursing credentialing to perform refugee health assessment. Prepares room, sterile instruments, equipment and supplies for examinations. Prepares patients for and assists with examinations or treatments. Performs phlebotomy and collects and processes other specimens for laboratory testing. Orders laboratory testing and performs TB skin or blood testing, reviews result and reads TST indurations, then counsels, interviews and educates clients regarding disease process, prevention, and treatment plan(s). Coordinates with other departments to complete wraparound services for clients including, but not limited to phlebotomy, immunizations, diabetes prevention and self-management educational services. Facilitates clinic flow to ensure timely services to clients.
