About the role
When someone has sought emergency medical care due to a mental health crisis, the hospital medical team may refer them to Crisis Diversion. This program helps individuals with: assistance navigating and accessing care systems and resource, development of comprehensive treatment and care plans, developing and repairing support systems and intensive case management. As an intensive case management program, it is designed to reduce unnecessary psychiatric hospitalizations by providing stabilization and linkage in the community.
Responsibilities
- Provide in-vivo supportive counseling to consumers and those involved in the consumer's network of care
- Link consumers to on-going behavioral health services and other related community supports/services as appropriate
- Provide on-going assessment of client's well being, clinical symptomology, behavioral changes and medication compliance
- Assist consumer's with ADL's through modeling and direct assistance
- Provide crisis intervention services as needed
- Other related duties as assigned
Requirements
- Bachelor's Degree in a Human Services field.
