Occupational Therapist (OT)

Requirements

• Graduated from an accredited Occupational Therapy (OT) college or university.
• CPR Certification.
• Unencumbered Occupational Therapy (OTR/L) license for New York.
• Conduct home visit Occupational Therapy assessments of referred patients providing direct patient care to assess medical conditions, functional capabilities, limitations, and potential for rehabilitation.
• Proficient in EMR documentation (we use Hello Note but experience in this system is not required)

Occupational Therapist Responsibilities:

• Conduct home visit Occupational Therapy assessments of referred patients.
• Provide direct patient care to assess medical condition, functional capabilities, limitations, and potential for rehabilitation.
• Establish and administer treatment programs with specific goals determined by the patient's capacity and tolerance, under the direction of the Physician.
• Set realistic and achievable goals for patients utilizing authorization, document and record their condition, and educate patients and families on the plan of care.
