Pharmacy Resident Match ProgramPOSITION SUMMARY
The pharmacy practice resident works closely with pharmacists and other medical professionals to expand upon and improve their clinical knowledge, as well as their professional and communication skills.
KEY RESPONSIBILITIES
- Works and develops collaborative relationships with pharmacists and other medical professionals.
- Completes rotations in ambulatory care, general medicine, infectious disease, nutrition and critical care.
- While in the clinics, meets with patients on a one-on-one basis to manage medication therapy, reviewing medication lists for inappropriate medication use, drug class duplication, drug interactions not previously recognized, and adverse effects.
- Works with patients to address knowledge deficits and overall medication adherence and all plans will be discussed with the pharmacy preceptor before implementation.
- While in the clinics, works directly with patients who have uncontrolled diabetes, hypertension, hyperlipidemia, asthma or COPD to help them reach their therapeutic goals.
- Staffs the Elliot Hospital inpatient pharmacy every third weekend for a total of eight hours each day on both Saturday and Sunday.
- Completes a residency research project and present results at the Eastern States Residency conference and to the pharmacy staff.
- Provides at least two educational inservices for physicians, two educational inservices for nurses and one educational inservice for pharmacists.
- Completes two journal clubs and present these to pharmacy staff.
- Completes a medication use evaluation project and present the results to the pharmacy staff.
- Provides drug information to patients and medical care providers upon request.
EDUCATION/EXPERIENCE/LICENSURE
Education: Pharm D. Degree required. Pharmacy internship preferred.
Certification/Licensure: Eligibility for pharmacy licensure exam. License to practice pharmacy to be obtained no later than 90 days after hire.
