Join our dedicated team at Hospice of the Piedmont as a full-time Registered Nurse Case Manager in southern Albemarle and surrounding southern counties to manage cases assigned and provide consistent, quality patient care. Duties include but are not limited to: 

  • Provide a nursing evaluation visit to new admissions.
  • Regularly re-evaluate patient needs.
  • Consult with the patient’s physician.
  • Complete bedside documentation using the EMR
  • Coordinate and manage services offered by health team members.
  • Help ensure the final chapter of your patient’s life is one of quality, compassion, and dignity.
  • Frequent travel required.

Qualifications:

  • The RN Case Manager position requires graduation from an accredited nursing school and a valid license as a Registered Nurse in the Commonwealth of Virginia; two years of relevant nursing experience is preferred.
  • Driver’s license, personal automobile, and current automobile liability insurance

Typical Physical Demands/Working Conditions:

  • The physical demands may require employee to regularly use hands to finger, handle, or feel objects, tools, or controls, reach with hands and arms; Sit, stand, stoop, bend, reach, lift and walk, and climb steps. Specific vision abilities required include close vision, distance vision, color vision, peripheral vision, depth vision, and the ability to adjust focus.
  • Communication by phone, writing, speaking; Regular use of computer screen and keyboarding are required
  • Lifting to 20 pounds, push up to 200 pounds in a wheelchair.
  • Drive short/long distances.
  • General fast-paced work environment
  • Physical and emotional health necessary for the accomplishment of duties
