About the role

Social Worker with primary role to support Specialty Clinics in various satellite locations in DE, PA and NJ.  Social Worker will be based in Wilmington and provide services from the hospital campus.  Secondary role will support inpatient general pediatrics as needed.  Role includes psychosocial screening and assessment and linkage to appropriate resources including support of social drivers of health, mental health and wellness and child welfare.  

Primary Responsibilities-

Manage complex assessments of the patient/family biopsychosocial situation; establish care plan goals and periodically reassess the patient/family situation as work progresses.

Attend patient care unit and/or medical division rounds in accordance with team expectations and provide inpatient admission screening on assigned patient care unit.

Provide crisis intervention services as indicated for the management of domestic violence, psychiatric emergency cases, child abuse, and family disputes.

Provide therapeutic, goal-directed counseling. Exhibit necessary clinical skills to provide problem-focused, goal-directed intervention. 

Interface with insurance providers, case managers, and others involved in the authorization of needed patient services. Identify and negotiate differing needs within the family system.

Coordinate discharge planning needs of patients in an efficient, effective, and timely manner while including the family in the process.

Establish and maintain effective working relationships across disciplines and coordinate family meetings. Assist in facilitating recommendations resultant of team meetings.

Document social work notes in the patient medical record according to Social Work Department policy. 

Provide community resources and information in response to assessed patient/family needs.

Maintain a professional manner at all times with families and Nemours associates.

Qualifications-

Master’s Degree in Social Work from a program accredited by the Council on Social Work Education (CSWE)

Active License to practice Social Work is required.
