As the Manager of Social Services & Alzheimer’s Unit, you will play a key role in supporting residents and families through advocacy, leadership, and relationship-centered care.

Your Purpose in This Role

You will lead Social Services, the Alzheimer’s Unit, and Activity programming while partnering closely with nursing and interdisciplinary teams. Your work will directly support residents’ emotional, social, and psychosocial well-being and strengthen the care experience for families.

How You’ll Serve

• Advocate for residents’ emotional, social, and psychosocial needs
• Partner with residents and families to create individualized, meaningful care plans
• Provide guidance and support during care transitions and discharge planning
• Educate residents and families on Medicare, Medicaid, and available assistance programs
• Oversee and complete Social Services MDS sections accurately and on time
• Collaborate with community agencies, consultants, and campus partners
• Provide leadership and oversight for the Alzheimer’s Unit with dignity-centered, person-focused care
• Foster a compassionate, mission-driven team culture
• Participate in care plan meetings, surveys, and quality improvement initiatives
• Identify opportunities to enhance services, processes, and the resident experience
• Mentor and support team members through training, feedback, and development

Qualifications

• Bachelor’s degree in social work or related field required
  (or Social Service Designee certification with plan to complete required coursework)
• 2–4 years of supervisory experience in healthcare, long-term care, or a related setting
• Long-term care experience and MDS knowledge preferred
