About our Team:
We employ a field-based, home-based care model and are committed to meeting members where they are--in their homes, in their community, and in our Hubs. You will go above and beyond to connect with Cityblock members in a non-judgmental, respectful and empathic manner, to meet their needs, and to provide feedback to the system as a whole as we strive to do better every day.
About the Role:
- In this role, you will continue to provide direct services to our members through our innovative care model.
- Your work will frequently be field-based, extending out in the surrounding neighborhood and community. It is integral to our care model that we meet people where they are, both physically and emotionally.
- As a Senior Community Health Partner, you will provide support to both our Community Health Partners (CHPs) and Community Health Partner leads.
- You will lend a helping hand to your Podmates with outreach, engagement and building Member Care Plans
- You will collaborate with our Hub-based leadership and Behavioral Health Specialists to support and review and advise on Hub wide performance metrics for our CHPs.
- You will partner with your CHP Lead and Pod’s clinical team to run daily huddles.
- You will lead Case Conference in your CHP lead’s absence.
- You will provide coaching and guidance to your Podmates.
- You will Provide guidance, coaching, and training around engagement, care planning, and longitudinal care.
- Care for your current panel of members; act as a point person for these members and partner with them longitudinally in attaining their goals.
- Support CHPs through their member outreach and engagement efforts.
- Model how to foster lasting and trusting relationships by doing so with your team and your members.
- Act as the back-up point person for the members on your team’s panels; co-visit members with CHPs, provide assistance in crises, etc.
- Support your team to work with members longitudinally to achieve their goals and identify new needs.
- Go above and beyond to connect with people in a non-judgmental, respectful and empathic manner, to meet their needs, and to provide feedback to the system as a whole as we strive to do better every day.
- You will manage Saturday Care as assigned on a rotating basis
Requirements for the Role:
- You are an experienced CHP with a deep knowledge of our care model, processes and procedures. You have expertise in one or more subject matter areas related to member care.
- You are a subject matter expert in one or more areas of care and with an appetite for sharing your knowledge across pods.
