Responsibilities
- The ideal candidate will advocate for resident well-being, support families through care transitions, and ensure compliance with all social service regulations and standards
- This position plays a vital role in enhancing residents' quality of life through assessment, counseling, care planning, and coordination of services
- Conduct psychosocial assessments and update care plans based on resident needs
- Provide counseling and supportive services to residents and families
- Assist with admission, discharge, and transfer processes, ensuring appropriate communication with physicians and families
- Facilitate discharge planning, including coordination with community resources and services
- Advocate for resident rights and provide education to staff on psychosocial issues
- Document social services interventions and progress in resident records
- Participate in interdisciplinary care conferences and survey preparation
- Ensure compliance with federal, state, and facility regulations related to social services
- Ability to move freely throughout the facility and interact with residents and families
- Occasional lifting of up to 40 lbs and pushing wheelchairs as needed
- Occasional evenings or weekends as needed
