Responsibilities

  • The ideal candidate will advocate for resident well-being, support families through care transitions, and ensure compliance with all social service regulations and standards
  • This position plays a vital role in enhancing residents' quality of life through assessment, counseling, care planning, and coordination of services
  • Conduct psychosocial assessments and update care plans based on resident needs
  • Provide counseling and supportive services to residents and families
  • Assist with admission, discharge, and transfer processes, ensuring appropriate communication with physicians and families
  • Facilitate discharge planning, including coordination with community resources and services
  • Advocate for resident rights and provide education to staff on psychosocial issues
  • Document social services interventions and progress in resident records
  • Participate in interdisciplinary care conferences and survey preparation
  • Ensure compliance with federal, state, and facility regulations related to social services
  • Ability to move freely throughout the facility and interact with residents and families
  • Occasional lifting of up to 40 lbs and pushing wheelchairs as needed
  • Occasional evenings or weekends as needed

Position Summary
