- Conduct comprehensive patient assessments and develop individualized care plans for home health clients
- Administer medications, wound care, IV therapy, and other skilled nursing treatments in the home setting
- Monitor and evaluate patient health status, documenting changes and communicating with physicians as needed
- Educate patients and family members on disease management, medications, and home safety
- Coordinate care with interdisciplinary team members including therapists, aides, and case managers
- Ensure accurate and timely clinical documentation in compliance with home health regulations
- Provide compassionate end-of-life and chronic disease management support as needed
- Active Registered Nurse (RN) license in the state of South Carolina
- Previous home health, hospice, or community nursing experience preferred
- Strong clinical assessment and critical thinking skills
- Excellent communication and patient education abilities
- Reliable transportation and a valid driver's license for home visits in the Hilton Head area
- Proficiency with electronic health records (EHR) and clinical documentation
- Compassionate, self-motivated, and able to work independently in a home care environment
