• Collaborate closely with onsite rehab and nursing teams to ensure coordinated care

• Track progress and maintain clear, timely documentation

🧠 What We’re Looking For:

• Active California PT license in good standing

• Solid clinical skills and a passion for patient-centered care

• Comfort with virtual platforms and remote engagement

• Great communication, adaptability, and tech-savvy approach

• Prior SNF or post-acute experience is a plus—but not required!
