About the role

Job Description:

The Community Paramedic evaluates and treats patients, usually in their own home based on a previous diagnosis and under the direction of advanced providers, offering clinical expertise and education within their specialty and areas of study. They collaborate with Tele Hospitalists, physicians, APPs, pharmacists, respiratory therapists, and patient placement teams to manage care of patients in their home environment and for transfers. The Community Paramedic is expected to be an extension of Intermountain’s Brick and Mortar services into the patients home, treating the patient for potentially advanced or complicated conditions in a place where they are most comfortable.

Essential Functions 

  • Treats patients based on their diagnosis and medical direction in the patients home 

  • Assist patients with medication administration and education 

  • Coordinates all mobile imaging, labs and orders 

  • Organize logistics for provider and admission order sets for Hospital Level Care at Home patients 

  • Clinical expert in problem solving for home equipment, meds and needs to ensure high level care is achieved 

  • Definitive resource for all ancillary services, providers, and patients 

  • Assist with education for patients and provides health-related educational opportunities 

  • Evaluates patients for various social determinants of health 

  • Builds standing relationships with patients who are seen in multiple successive days

Minimum Qualifications 

  • Successful completion of Paramedic course 

  • Must obtain Paramedic license in state of operation within 120 days 

  • Basic Life Support (BLS) for Healthcare Providers 

  • Current valid driver license in good standing 

  • Three years of clinical care experience. 

 

Preferred Qualifications  

  • Advanced Trauma Life Support (ATLS), Transport Professional Advanced Trauma Course (TPATC) or Pre-Hospital Trauma Life Support (PHTLS) Certification  

  • Neonatal Resuscitation Program (NRP) Certification 

  • Paramedic National Registr
